New Patient Checklist

Welcome! We are so happy you have decided to take the first step in the healing process with us!

Below you will find the steps to take as a new patient:

1. Use the link to the right to book a new patient appointment.

   You can also book an appointment over the phone if you prefer (640-249-8888).

2. Wait for confirmation.

   Usually within 24-48 hours, your appointment will be approved and you will receive confirmation by email.

3. Fill out patient forms.

   Once your appointment is approved, you will receive links to patient forms on Practice Better by email.

4. Complete the Symptom Survey.

   Click on the link to the right and complete the survey. Once you finish, download the results and save them to your computer. You can then upload the results to your patient portal on Practice Better (link below) or print the results and bring them with you to your appointment.
